Installation is very easy,

1) Go to the root of the Quake 4 game directory
2) copy the batch files into this root directory
3) copy the guru5.demo file towards the q4base\demos\ directory (you need to create the demos directory)

In Quake 4 select your preferred (HQ) configuration and with your graphics card driver properties you can force AA and AF modes.

demo-10x7.bat start up the benchmark sequency for a test at 1024x768
demo-12x10.bat start up the benchmark sequency for a test at 1280x1024
demo-16x12.bat start up the benchmark sequency for a test at 1600x1200
